Advance Practice Clinician to conduct Annual Health and Wellness Assessments (AHWA) on Presbyterian Medicare Advantage members in New Mexico Responsibilities:
*Conduct Annual Health and Wellness Assessments (AHWA) on Presbyterian Medicare Advantage in assigned territory statewide (New Mexico).
*During AHWA complete a personalized physical examination, H&P, vital signs, medication reconciliation, and document current mental and social condition. Assessment does not include prescribing or treatment of members.
*Serve as health care resources and patient advocate. Including education and counseling of members on conditions identified and making recommendations for screening procedures.
* Complete documentation in the EMR in appropriate and timely manner.
*Communicate member healthcare issues to primary care provider and/or PHP for continued management.
*Actively participate in piloting improvement projects, organizational initiatives, team building exercises, section meetings, and semi-annual provider reviews.
*Performs additional duties as directed. Credentials Required
For Nurse Practitioner - Masters degree in Nursing, New Mexico Nurse Practitioner license, federal and state DEA license and ANCC certification required. Minimum of one year experience preferred but not required
